Baton Rouge General Medical Center announced a new partnership with Lafayette-based LHC Group Inc. this morning by which LHC Group will serve as the preferred home health care partner for all of Baton Rouge General’s patients. Financial terms of the partnership were not disclosed.
The hospital says the partnership will help patients seamlessly transition “from hospital to home.” It will also create a Baton Rouge office for LHC Group on the hospital’s Mid City campus.
“Working together, we can help more patients recover in the comfort of their own homes, and avoid returning to the hospital unnecessarily,” says Mark Slyter, Baton Rouge General’s president and CEO, in a statement.
The announcement follows the unveiling of the hospital system’s long-term plan for its Bluebonnet and Mid City campuses in November. The hospital’s “Future of Healthcare” plan calls for increased collaboration with other health care providers to improve health, convenience and the affordability of care. The report also notes the hospital system’s intent to explore partnerships with “like-minded” partners.
LHC Group provides post-acute healthcare services to patients within their homes through hospice, home health, community-based agencies and long-term acute care hospitals.
“Hospital and home health partnerships are one key component of helping patients manage their conditions,” reads a press release on the partnership.
Patients with certain conditions are referred to home health as part of recovery. Physicians then work with home care team to help seamlessly transition patients back into daily life. Home health professionals also will educate patients on their conditions, nutrition and medication management.
